This list provides the documentation FourPoint Energy will need in order to complete a change to your ownership account. All change requests must be submitted in writing.

IRS W-9 Form

Federal law requires all owners to provide their social security number or tax identification number via an IRS W-9 Form. Should you fail to provide this information, you may become subject to non-refundable backup withholding.

    • Marriage Certificate or Divorce Decree reinstating maiden name or other legal document of name change

    • Plan of reorganization
    • Order confirming the plan of reorganization
  5. CHANGES IN TRUSTEE

    • Instrument by which the Successor Trustee was appointed
  6. COMPANY NAME CHANGE

    • Certificate of Name Change including Tax I.D.
    • New IRS W-9 Form
  7. CHANGE DUE TO MERGER

  8. DEATH OF OWNER WITH A WILL WITH PROBATE PROCEEDINGS CONDUCTED IN THE STATE WHERE THE PROPERTY IS LOCATED

    • Death Certificate
    • Letters testamentary
    • Final decree of distribution or judgment of possession
  9. DEATH OF OWNER WITH A WILL WITH PROBATE PROCEEDINGS CONDUCTED OUTSIDE THE STATE WHERE THE PROPERTY IS LOCATED

    • Death Certificate
    • Letters testamentary
    • Final decree of distribution or judgment of possession recorded in the county where the property is located
    • Documentation of ancillary probate proceedings opened in all affected states
  10. DEATH OF OWNER
    WITHOUT A WILL

    • Death Certificate
    • Letters of administration
    • Final decree of distribution or judgment of possession recorded in the county where the property is located
  11. ENROLL OR CHANGE IN REVENUE DIRECT DEPOSIT

  12. JOINT TENANCY WITH DEATH OF ONE TENANT

    • Death Certificate of deceased tenant Life Estate – Death
  13. Life tenant’s Death Certificate

    • Names and addresses of remaindermen
  14. NAME CHANGE

    • Marriage Certificate or Divorce Decree reinstating maiden name or other legal document of name change
  15. Ownership Change
    Due to Divorce

    • Complete Divorce Decree including Settlement Agreement and recorded conveyances (if applicable)
  16. Sale of Interest

    • Complete copy of conveyance document filed of record in the county which the property is located
  17. Trust or Partnership Creation or Dissolution

    • Trust or Partnership Agreement and recorded conveyance(s) (if applicable)
